In this expression, the adversary is using part of a person or animal, the skin, to mean all of that person or animal. The basic meaning is that a person will sacrifice even a valuable herd animal in order to save himself, and the more general meaning, as the adversary explains in the rest of the verse, is that a person will sacrifice just about anything he owns in order to stay alive.
