OET (OET-LV) And all the messengers had_stood around the throne, and the elders, and the four living_creatures, and they_fell before the throne on the faces of_them, and they_prostrated the before_god
OET (OET-RV) Then all the messengers stood around the throne, and around the elders and the four living beings. The messengers all knelt down in front of the throne with their faces to the ground and worshipped God
In this section, John saw a new part of this vision. He saw believers rejoicing, praising, and worshiping in heaven. One of the elders around the throne (4:4) explained the vision to John.
Other examples of headings for this section are:
The vision of a huge crowd of believers and its explanation
The Great Multitude in White Robes (NIV)
God’s People Around His Throne in Heaven (GW)
And all the angels stood around the throne
All the angels were standing around the throne, (NIV)
All of the angels were standing in a circle around the place/seat of ruling,
And all the angels stood around the throne and around the elders and the four living creatures: The throne was at the center of the area. Near the throne were the four living creatures (4:6b). The elders were in a circle around the throne and the four living creatures (4:4a). This clause indicates that the angels surrounded all these things. Other ways to translate this clause are:
All the angels stood around the throne, the elders, and the four living creatures. (GNT)
all the angels who were standing in a circle round the throne, surrounding the elders and the four living creatures (NJB)
and around the elders and the four living creatures.
the elders, and the four living beings.
and surrounded the leaders and the four living beings.
the elders: Here the word elders refers to those who have authority. These twenty-four people (or angels) were leaders. They had God as their leader. See how you translated this word in 4:4.
the four living creatures: The Greek word that the BSB translates as living creatures is literally “living (ones).” The word refers to beings that are like angels. See how you translated this phrase in 4:6.
And they fell facedown before the throne and worshiped God,
They prostrated themselves before the throne and worshiped/honored God,
They knelt and put their faces to the ground with their arms outstretched before the place/seat of ruling and revered God.
And they fell facedown: Here, this phrase refers to kneeling and putting the face to the ground with arms outstretched toward someone. This position showed complete humility before God and honored him. Other ways to translate this phrase are:
prostrated themselves…and touched the ground with their foreheads (NJB)
threw themselves down with their faces to the ground (NET)
Many languages have a different action that shows complete humility and honor to someone. If that is true in your language, you may want to use that word or phrase. See how you translated the similar phrases in 4:10 or 5:14.
worshiped: This word refers to honoring someone or something as sacred/divine. Other ways to translate this word are:
venerated
revered

ἔπεσαν & ἐπὶ τὰ πρόσωπα αὐτῶν
˱they˲_fell & (Some words not found in SR-GNT: Καί πάντες οἱ ἄγγελοι εἱστήκεισαν κύκλῳ τοῦ θρόνου καί τῶν πρεσβυτέρων καί τῶν τεσσάρων ζῴων καί ἔπεσαν ἐνώπιον τοῦ θρόνου ἐπί τά πρόσωπα αὐτῶν καί προσεκύνησαν τῷ Θεῷ)
Be sure that it is clear in your translation that the angels and elders and living creatures did not fall down accidentally. They bowed down in front of the throne as a gesture to honor God. Alternate translation: [they bowed down with their faces to the ground]
7:1-17 Three interludes occur in chs 6–14 (ch 7; 10:1–11:14; 12:1–14:20) to define the place of God’s holy people and to provide perspective on the previous scenes. In this first interlude before the seventh seal is broken, two visions communicate how God protects his people and assures them of his calling.

Note: The OET-RV is still only a first draft, and so far only a few words have been (mostly automatically) matched to the Hebrew or Greek words that they’re translated from.
