This is symbolic language to indicate that the angel was sent from Yahweh in heaven to judge the people on the earth.

(Occurrence 0) having a drawn sword in his hand raised over Jerusalem
(Some words not found in UHB: and,lifted Dāvid DOM eyes_of,his and=he/it_saw DOM messenger_of YHWH standing between the=earth/land and=between the=heavens and,sword_of,his drawn in=his/its=hand stretched_out on/upon/above/on_account_of//he/it_went_in Yərūshālam/(Jerusalem) and,fell Dāvid and,the,elders clothed in/on/at/with,sackcloth on/upon/above/on_account_of//he/it_went_in faces_of,their )
The angel holding a sword in his hand is a symbolic action that shows he is ready to attack Jerusalem. This is symbolic, because the judgment was sickness. Alternate translation: “holding a sword in his hand as if ready to attack Jerusalem”
Note 3 topic: translate-symaction
(Occurrence 0) clothed in sackcloth, lay facedown on the ground
(Some words not found in UHB: and,lifted Dāvid DOM eyes_of,his and=he/it_saw DOM messenger_of YHWH standing between the=earth/land and=between the=heavens and,sword_of,his drawn in=his/its=hand stretched_out on/upon/above/on_account_of//he/it_went_in Yərūshālam/(Jerusalem) and,fell Dāvid and,the,elders clothed in/on/at/with,sackcloth on/upon/above/on_account_of//he/it_went_in faces_of,their )
These were symbols of repentance.
21:16-17 These verses provide a more detailed description of the angel and of David’s response than the parallel text (2 Sam 24:17). The sight of the angel carrying out his mission inspired David to plead on behalf of the people.

Note: The OET-RV is still only a first draft, and so far only a few words have been (mostly automatically) matched to the Hebrew or Greek words that they’re translated from.
