Here Isaiah is speaking as if the people’s anguish were literally that of a woman who has been pregnant and has writhed in labor. Your language may have a comparable expression that you can use in your translation, or you could state the meaning plainly. Alternate translation: [We have toiled greatly, we have been in great pain]

כְּמ֖וֹ יָלַ֣דְנוּ ר֑וּחַ

but gave_birth wind

The point of this comparison is that just as having birthed wind would produce nothing, so also the people’s suffering has been futile and has not accomplished deliverance. Alternate translation: [as if our labor had produced nothing]
