Here Yahweh is speaking of remembering Zion permanently as if she were something he had inscribed on his hands, saying on my palms I have inscribed you. Your language may have a comparable expression that you can use in your translation, or you could state the meaning plainly. Alternate translation: [I will never forget you]

In verses 16-23, the word you is feminine singular because it refers to Zion, which is being personified as a woman. So use the singular form in your translation if your language marks that distinction, and use the feminine form if your language marks that distinction.
