The word translated as when indicates that two things happen at the same time. The overflowing scourge passes through, and at that same time, the leaders will be trampled by it. Use a connector in your language that makes it clear that these things happen at the same time. Alternate translation: [as it passes through]

Isaiah is using you will be to it for trampling as a common expression of the culture to mean “it will trample you.” If this phrase does not have that meaning for your readers, you could use a comparable expression from your language that does have that meaning, or you could state the meaning plainly.
