These two clauses, with his mouth, they bless and in their inside, they curse, mean basically opposite things. The psalmist is contrasting what his enemies say outwardly with what they feel inwardly. Hebrew poetry was based on this kind of repetition and contrast, and you may want to show this to your readers by including both clauses in your translation. If it would be helpful in your language, you could make the contrast more explicit. Alternate translation: [they say kind things, but they really wish harm]
