Here David is speaking of himself as if he were a worm. He means that other people treat him as if he were worthless and do not care if he lives or not. If it would be helpful, you could use a comparable expression from your language or state the meaning plainly. Alternate translation: [But I am treated as if I were a worthless worm and not a human]

By a reproach of mankind, David means that he has become someone whom all people mock and shame. You could indicate this explicitly in your translation if that would be helpful to your readers. Alternate translation: [someone whom people mock] or [an object of shame to everyone]

The expression despised by the people is a passive verbal form. If it would be unnatural to use that form, you could express the idea in active form or in another way that is natural in your language. Alternate translation: [and the people despise me]
